Schoolhouse Pediatrics is pleased to announce the launch of their all new PatientPortal. This online service will allow you to keep track of your child(ren)’s personal health information (ie. immunization records, annual physicals, lab reports, etc.), update demographic information, request an appointment or prescription refill as well as communicate with our practice regarding billing, medical inquiries and general office questions.
To request authorization, please complete the form below. Then, be on the lookout for an email from ChartMaker® PatientPortal with instructions on how to complete the registration. Be sure to check your junk/spam mail folders for registration email if you have not received within 24 hours of submitting your request.
