Welcome to the Schoolhouse Pediatrics Family! We’re so happy you have chosen our practice for the pediatric needs of your child(ren). Prior to your first visit, you will be required to fill out our New Patient Packet (one for each child) and Transfer of Medical Records forms which can be found under Patient Forms and General Practice Forms. If you have any questions about either form, please do not hesitate to contact our office.
